Astin-CV (Atorvastatin + Clopidogrel)

• Composition :
  Each capsule contains Atorvastatin 10mg / 20mg and Clopidogrel 75mg.
• Indications :
  For the treatment of dyslipidemia associated with atherosclerotic arterial disease with the risk of myocardial infarction, stroke or peripheral vascular disease.
• Pharmacodynamics :
  Atorvastatin is a selective, competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme that converts 3-hydroxy-3methylglutaryl-coenzyme A to mevalonate, a precursor of sterols, including cholesterol. With ultracentrifugation, these complexes separate into HDL, IDL, LDL, and VLDL fractions. Clopidogrel is a platelet aggregation inhibitor. Clopidogrel is metabolized by CYP450 enzymes to produce the active metabolite that inhibits platelet aggregation. The active metabolite of clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor and the subsequent ADP-mediated activation of the glycoprotein GP IIb/IIIa complex, thereby inhibiting platelet aggregation.
• Dosage and administration :
  One tablet, once daily.
• Contraindications :
  Contraindicated in hypersensitivity to the drug substance or any component of the product, pathological bleeding such as peptic ulcer or intracranial hemorrhage.
• Precautions and warnings :
  Atorvastatin - Myopathy or having a risk factor predisposing to the development of renal failure secondary to rhabdomyolysis, Liver Dysfunction, patients with recent stroke or TIA, endocrine function, hepatic impairment. Clopidogrel- Prolongs bleeding time and therefore should be used with caution in patients who may be at risk of increased bleeding.
• Use in pregnancy, lactation & pediatrics :
  Contraindicated in women who are or may become pregnant. Not recommended during nursing. Safety and effectiveness has not been established in pediatrics.
• Drug interactions :
  Atorvastatin- caution should be exercised, when administered with strong inhibitors of CYP 3A4, clarithromycin, combination of protease inhibitors, itraconazole, cyclosporine, digoxin, oral contraceptives and rifampin. Clopidogrel- Avoid concomitant use with omeprazole. NSAIDs and warfarin should be co-administered with caution.
• Adverse reactions :
  The most common adverse events Atorvastatin- constipation, flatulence, dyspepsia and abdominal pain. Clopidogrel- Bleeding, including life-threatening and fatal bleeding, is the most commonly reported adverse reaction.
