  • Therapy with esomeprazole does not preclude the presence of gastric malignancy. Avoid concomitant use of esomeprazole/omeprazole with clopidogrel. Inhibition of platelet aggregation by clopidogrel is entirely due to an active metabolite. PPI therapy may be associated with increased risk of Clostridium difficile associated diarrhea. Atrophic gastritis has been noted with long-term omeprazole therapy. Bone Fracture: Long-term and multiple daily dose PPI therapy may be associated with an increased risk for osteoporosis-related fractures of the hip, wrist or spine.
  • Pregnancy: Pregnancy Category C. Esomeprazole is likely present in human milk. Caution should be exercised when administered to a nursing woman. Safety and effectiveness of esomeprazole have not been established in patients less than 1 month of age.
  • Esomeprazole may affect plasma levels of antiretroviral drugs – use with atazanavir and nelfinavir is not recommended. Esomeprazole interfere with drugs for which gastric pH affects bioavailability (e.g., ketoconazole, iron salts, erlotinib, digoxin and mycophenolate mofetil).
  • The most common adverse effects are headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth.
